How to Order
Please fax or email your purchase order and be sure to reference a valid written quotation number issued by Polyphase Microwave Inc. Payment terms are NET 30 for customers with approved credit. We also accept credit card and C.O.D. orders.
Terms & Conditions
All sales are subject to Polyphase Microwave Inc.’s standard terms and conditions 
(11 KB), unless specifically agreed to otherwise by the factory. Shipments are F.O.B. Bloomington, Indiana, U.S.A.
Delivery
When a carrier is not specified in the body of the purchase order, shipment will be made via UPS-BLUE service prepaid and added to the invoice.
Insurance
Unless requested otherwise at the time an order is placed, all shipments will be insured at full value. The insurance charge will be added to the invoice.
